ABSTRACT

En las consultas de cirugía general se reciben, con frecuencia, pacientes portadores de hernias de la pared abdominal y, dentro de ellas, las hernias de la región inguinal. La hernia, como entidad, se menciona desde el año 1500 a. C. en el papiro Ebers. Su incidencia es alta, con una anatomía compleja, y en ocasiones se plantean porcentajes de recidivas frecuentes, por lo que hace de esta entidad un interesante tema con gran valor para los cirujanos en general. El objetivo de este trabajo es considerar que la técnica de reparación herniaria de Mohan Desarda cumple con los principios fundamentales de reparación de la hernia. En ella las complicaciones son mínimas debido a la utilización de una franja de tejido autólogo como refuerzo para el cierre del defecto, y el índice de recidiva es menor en comparación con el demostrado por otras.


In general surgery consultations, patients carrying hernias of the abdominal walls, and within them, hernias of the abdominal wall, are frequently received. The hernia, as an entity, has been mentioned since the year 1500 BC C. on the Ebers papyrus. Its incidence is high, with a complex anatomy, and sometimes percentages of frequent recurrency are stated, which makes this entity an interesting topic of great value for surgeons in general. The main objective of this work is to consider that Mohan Desarda's hernia repair technique complies with the main principles of hernia repair. In it, complications are minimal due to the use of a strip of autologous tissue as reinforcement for closing the defect, and the recurrence index is lower compared to that demonstrated by others.

Introducción: Las hernias de la pared abdominal afectan entre el 10% al 15% de la población mundial, siendo hasta el 60% de estas hernias inguinales. Las hernias inguinales gigantes son poco comunes, pero con una gran carga de enfermedad para el paciente. Caso Clínico: Se presenta el caso de un paciente de 51 años, con antecedente de diabetes mellitus tipo II, proveniente de zona rural aislada, por cuatro días de evolución consistente en quemadura escrotal por metal caliente, relacionada a una hernia inguinoescrotal derecha gigante. Es llevado a intervención quirúrgica por cirugía general y urología. Por adecuada evolución clínica se da de alta al 5° día posoperatorio. Discusión: Las hernias inguinales gigantes son raras y frecuentemente se presentan en pacientes de bajo estrato socioeconómico, procedencia rural y cierto grado de negligencia. El reto del equipo quirúrgico consiste en lidiar con los posibles efectos adversos de la reducción del contenido herniario en un abdomen con diversos grados de pérdida del dominio. Se puede requerir resección o debulking del contenido abdominal o la expansión de la cavidad abdominal mediante frenectomía, neumoperitoneo progresivo perioperatorio o la creación de hernias ventrales mediante maniobras avanzadas. La reparación con malla libre de tensión disminuye el riesgo de recurrencia. Conclusión: La hernia inguinal gigante es una patología rara. El cirujano general está llamado a conocer el abanico de opciones que existen en caso de enfrentarse a estos pacientes, lo cual ayuda a reducir la elevada morbimortalidad y altas tasas de recurrencia.


Introduction: Abdominal wall hernias affect between 10% to 15% of the world population and up to 60% of these are inguinal hernias. Giant inguinal hernias are rare, but have high burden of disease for the patients. Clinical Case: We present the case of a 51-year-old patient, with a history of type II diabetes mellitus, from an isolated rural area, with four days of a scrotal burn by hot metal, related to a giant right inguinoscrotal hernia. He is taken to surgical intervention by general surgery and urology. Due to adequate clinical evolution, he was discharged on the 5th postoperative day. Discussion: Giant inguinal hernias are rare and frequently occur in patients of low socioeconomic status, rural origin and a certain degree of neglect. The challenge for the surgical team consist in dealing with the potential adverse effects of reducing hernia contents in an abdomen with varying degrees of loss of normal capacity. Resection or debulking of the abdominal contents or expansion of the abdominal cavity by frenectomy, perioperative progressive pneumoperitoneum, or the creation of ventral hernias by advanced maneuvers may be required. Tension-free mesh repair decreases the risk of recurrence. Conclusion: Giant inguinal hernia is a rare pathology. The general surgeon is called to know the range of options that exist in the event of facing these patients, which helps to reduce the high morbidity and mortality and high rates of recurrence.

ABSTRACT

Las hernias inguinales son una patología para tratamiento quirúrgico frecuente, afecta entre un 3 a 5 % de toda la población en general, su reparación resulta difícil y peligrosa en caso de recurrencia; un paciente sometido a cirugía tiene entre19 ­ 39 % de probabilidades de recidiva, y esto aumenta con cada nuevo procedimiento. Objetivo: caracterizar la prevalencia de recidivas de hernias inguinales en pacientes intervenidos quirúrgicamente en el Servicio de Cirugía General del Hospital Universitario de Caracas entre los años 2015-2020. Métodos: estudio retrospectivo, descriptivo y analítico. Resultados: se estudiaron 52 casos de recidivas de hernias inguinales, 43 % de un total de 120 pacientes sometidos a hernioplastia inguinal, las técnicas quirúrgicas con mayor número de recidivas fueron: la de Bassini con 14 %, la de Rutkow-Robbins con 10 % y la de Linchtenstein con 9 % de los casos; con un tiempo de recidiva posterior a la primera intervención el cual fue mayor al año de la primera intervención. Conclusiones: la recidiva resultó un poco mayor al promedio, el tiempo en el cual ocurrió es mayor al año de la intervención y las técnicas más involucradas fueron, enorden decreciente: Bassini, Rutkow-Robbins, y Linchtenstein(AU)


Inguinal hernias are a pathology for frequent surgical treatment, affecting between 3 to 5 % of the entire population in general, their repair is difficult and dangerousin case of recurrence; a patient undergoing surgery has a19-39 % chance of recurrence, and this increases with eachnew procedure. Objective: to characterize the prevalence of recurrences of inguinal hernias in patients undergoing surgery at the General Surgery Service of the Hospital Universitario de Caracas between the years 2015-2020. Introducción: Las intervenciones quirúrgicas de hernias son uno de los procedimientos que más frecuente realizan los cirujanos. Objetivo: Caracterizar a los pacientes a los que les fue realizada la técnica quirúrgica de hernioplastia inguinal según la técnica de Jean Rives modificada. Métodos: Se realizó un estudio descriptivo retrospectivo de corte longitudinal en el Hospital Universitario Arnaldo Milián Castro de Santa Clara en el período de enero del 2011 a diciembre del 2021. El universo de trabajo estuvo representado por la población de pacientes con el diagnóstico de hernias inguinocrurales, a los cuales les fue efectuada dicha técnica quirúrgica. La muestra estuvo conformada por 194 pacientes. Resultados: Predominó el sexo masculino 103 (53,1 por ciento) y los pacientes mayores de 60 años 99 (51,0 por ciento). La localización más frecuente de la hernia fue la zona inguinal derecha 146 (75,3 por ciento) para ambos sexos, masculino 81 (41,8 por ciento) y femenino 65 (33,5 por ciento) respectivamente. Prevalecieron las hernias inguinales primarias 96 (49,5 por ciento), directas 70 (36,1 por ciento); en el sexo masculino preponderaron las indirectas 61 (31,4 por ciento). Predominaron las complicaciones posoperatorias en las cirugías de urgencias 10 (5,0 por ciento). El seroma 4 (2,0 por ciento), la orquitis 3 (1,5 por ciento) y la recidiva herniaria 3 (1,5 por ciento) fueron las complicaciones más comunes. Conclusiones: La técnica de hernioplastia inguinocrural de Jean Rives modificada es efectiva para los pacientes masculinos mayores de 60 años con hernias inguinales primarias, directas e indirectas. Las posibles complicaciones posoperatorias fueron en las intervenciones quirúrgicas de urgencia, donde el seroma y la orquitis fueron las más frecuentes. La recidiva tuvo una baja tasa con respecto a otros tipos de hernioplastias(AU)


Introduction: Hernia surgical interventions are one of the procedures most frequently performed by surgeons. Objective: To characterize the patients who underwent the surgical technique of inguinal hernioplasty based on the modified Jean Rives technique. Methods: A retrospective, descriptive and longitudinal study was carried out in Hospital Universitario Arnaldo Milián Castro, of Santa Clara City, central Cuba, in the period from January 2011 to December 2021. The study universe was represented by the population of patients diagnosed with inguinocrural hernias who underwent this surgical technique. The sample consisted of 194 patients. Results: There was a predominance of the male sex, accounting for 103 (53.1 percent); as well as of patients over 60 years of age, accounting for 99 (51.0 percent). The most frequent location of the hernia was the right inguinal area, represented by 146 cases (75.3 percent) for both sexes, accounting for 81 (41.8 percent) and 65 (33.5 percent) for males and females, respectively. Primary inguinal hernias predominated, represented by 96 cases (49.5 percent); as well as direct hernias, accounting for 70 cases (36.1 percent). In the male sex, indirect hernias predominated, accounting for 61 cases (31.4 percent). Postoperative complications predominated in emergency surgery, represented by 10 cases (5.0 percent). Seroma (4; 2.0 percent), orchitis (3; 1.5 percent) and hernia recurrence (3; 1.5 percent) were the most common complications. Conclusions: The modified Jean Rives inguinocrural hernioplasty technique is effective for male patients older than 60 years with primary, direct and indirect inguinal hernias. Potential postoperative complications occurred in emergency surgery, in which cases seroma and orchitis were the most frequent ones. Recurrence had a low rate compared to other types of hernioplasties(AU)

Resumen Introducción: El procedimiento de neumoperitoneo progresivo preoperatorio para el manejo de hernias gigantes con "Pérdida de dominio" o Pérdida de derecho a domicilio" fue introducido en 1940 por Goñi Moreno en Argentina. Autores como Herszage, Berlemont, Koontz, Gravez y Martínez Munive presentaron variaciones interesantes del método original. Su uso se recomienda para preparación previa de pacientes con hernias gigantes y grandes contenidos de vísceras en el saco herniario, en los cuales no sería posible su re-introducción y efectuar la hernioplástia, o en los que su reducción de manera forzada pudiera llevar al paciente al desarrollo de un síndrome compartimental abdominal en el postoperatorio inmediato. Caso clínico: Paciente de sexo masculino de 65 años de edad, que presenta cuadro clínico de un mes de evolución caracterizado por dolor a nivel de la región inguinal izquierda, en el cual se evidencia presencia de hernia inguino escrotal gigante izquierda, no reductible, con aproximadamente un 40% de contenido abdominal. Resultados: La mayoría de los estudios describen el uso del neumoperitoneo progresivo para la reparación de las eventraciones gigantes, sin embargo, en nuestra experiencia y en la de otros autores, esta técnica también puede utilizarse para resolver las hernias inguinales y umbilicales gigantes, con buenos resultados. En todos los casos se aconseja utilizar material protésico. Conclusión: El uso del neumoperitoneo preoperatorio progresivo, se trata de una técnica segura y fácil de realizar que puede complementar las técnicas de eventroplastía complejas, aportando ventajas en la preparación de los pacientes con grandes defectos de pared abdominal y obteniendo buenos resultados.


Abstract Introduction: The procedure of progressive preoperative pneumoperitoneum for the management of giant hernias with "Loss of domain" or "Loss of the right of domain" was introduced in 1940 by Goñi Moreno in Argentina, followed in later years by authors such as Herszage, Berlemont, Koontz, Gravez and Martínez Munive, all with some interesting variations of the original method. Its use is recommended for previous preparation of patients with giant hernias and large contents of viscera in the hernial sac, in which it would not be possible to re-introduce and perform hernioplasty, or in which its forced reduction could lead to the patient to the development of an abdominal compartment syndrome in the immediate postoperative period. Clinical case: A 65-year-old male patient with a clinical picture of one month of evolution characterized by pain in the left inguinal region. There was evidence of a giant left inguinal scrotum, not reducible, with approximately 40% abdominal contents. Results: Most studies describe the use of progressive pneumoperitoneum for the repair of giant hernias, however, in our experience and in that of other authors, this technique can also be used to resolve hernias giant inguinal and umbilical, with good results. In all cases it is recommended use prosthetic material. Conclusion: The use of progressive preoperative pneumoperitoneum is a safe and easy-to-perform technique that can complement complex eventroplasty's techniques, providing advantages in the preparation of patients with large abdominal wall defects and obtaining good results.

RESUMEN La hernia vesical es una entidad asociada a la hernia inguinal, con predisposición de lado derecho, en un porcentaje de 0,5 a 3%, hasta 10% en hombres, y a partir de la quinta a séptima década de vida1. La mayoría son pequeñas; la fisiopatología más común es la obstrucción mientras que la hiperplasia prostática es la principal etiología. La presentación clínica es poco específica, y el diagnóstico es en la mayoría de los casos transoperatorio. Se presenta el caso de un paciente de género masculino de 64 años, con antecedente patológico de enfermedad de Parkinson, hernia inguinal izquierda reducible hace 5 años. Acudió a consulta médica por manifestar dolor abdominal de moderada intensidad, más aumento de volumen en región inguinoescrotal izquierda. Al realizar el examen físico se constató una hernia inguinoescrotal izquierda no reducible. Con el diagnóstico de hernia inguinal incarcerada se realizó una exploración quirúrgica, con hallazgos de hernia inguinoescrotal de gran tamaño con contenido vesical y epiplón incarcerado con cambios de coloración. Se realizó entonces la reparación de la hernia. La evolución posoperatoria fue satisfactoria sin complicaciones.


ABSTRACT Bladder hernia is a condition associated with inguinal hernia, usually right-sided, in 0.5 to 3% of the cases and up to 10% in men between the fifth and seventh decade of life. Most hernias are small; the most common pathophysiology is obstruction while prostatic hyperplasia is the main etiology. The clinical presentation is unspecific, and the diagnosis is usually made during surgery. We report the case of a 64-year-old male patient with a history of Parkinson's disease and reducible left inguinal hernia 5 years before, who sought medical advice due to abdominal pain of moderate intensity, with increased volume in the left inguinoscrotal region.On physical examination a diagnosis of left-sided non-reducible inguinoscrotal hernia was made. With the diagnosis of incarcerated inguinal hernia the patient underwent surgical exploration which showed a large inguinoscrotal hernia containing the bladder and incarcerated omentum with color changes. The hernia was repaired, and the patient evolved with favorable outcome.

RESUMEN La hernia inguinal es una enfermedad frecuente no exenta de complicaciones. Con el objetivo de determinar la prevalencia de las complicaciones posquirúrgicas en pacientes con hernia inguinal; se realizó un estudio cuantitativo, transversal y descriptivo en un universo de 86 pacientes intervenidos quirúrgicamente por hernia inguinal en el año 2019; Hospital General Mariano Pérez Balí, Granma. Se evaluaron las variables: edad, sexo, tipo de hernia inguinal, lateralidad, técnica quirúrgica y complicaciones constatadas. En un total de 86 pacientes con hernia inguinal la edad media fue de 53,81 años. El sexo masculino representó el 95,34 % del total de pacientes, las hernias tipo II de Nyhus representaron el 46,24 %. Las técnicas de hernioplastias fueron empleadas en la reparación del 54,84 % del total de hernias reparadas particularmente la técnica de Lichtenstein. El 66,66 % de las hernias inguinales sometidas a reparación quirúrgica no presentaron complicaciones mientras que el 11,83 % se complicaron de forma aguda con funiculitis. Se concluye que la incidencia de las complicaciones posquirúrgicas en la hernia inguinal fue elevada.


ABSTRACT Inguinal hernia is a common disease not free of complications. In order to determine the prevalence of postoperative complications in patients with inguinal hernia; a quantitative, cross-sectional and descriptive study was carried out in a universe of 86 patients who underwent surgery for inguinal hernia in 2019; Mariano Pérez Balí General Hospital, Granma. The variables were evaluated: age, sex, type of inguinal hernia, laterality, surgical technique and verified complications. In a total of 86 patients with inguinal hernia the mean age was 53,81 years. Male sex represented 95,34 % of all patients, Nyhus type II hernias represented 46,24 %. Hernioplasty techniques were used to repair 54,84 % of all hernias repaired, particularly the Lichtenstein technique. The 66,66 % of the inguinal hernias undergoing surgical repair did not present complications while 11,83 % were acutely complicated with funiculitis. It isconcluded that the incidence of postoperative complications in inguinal hernia was high.


RESUMO A hérnia inguinal é uma doença frequente que não está isenta de complicações. A fim de determinar a prevalência de complicações pós-operatórias em pacientes com hérnia inguinal; foi realizado um estudo quantitativo, transversal e descritivo em um universo de 86 pacientes submetidos à cirurgia de hérnia inguinal em 2019; Hospital Geral Mariano Pérez Balí, Granma. Foram avaliadas as variáveis: idade, sexo, tipo de hérnia inguinal, lateralidade, técnica cirúrgica e complicações encontradas. Em um total de 86 pacientes com hérnia inguinal, a média de idade foi de 53,81 anos. O sexo masculino representou 95,34% do total de pacientes, as hérnias tipo II de Nyhus representaram 46,24%. As técnicas de hernioplastia foram utilizadas no reparo de 54,84% de todas as hérnias reparadas, principalmente a técnica de Lichtenstein. 66,66% das hérnias inguinais foram submetidas a correção cirúrgica sem complicações, enquanto 11,83% foram agudamente complicadas com funiculite. Conclui-se que a incidência de complicações pós-operatórias em hérnia inguinal foi alta.

ABSTRACT

Introducción: Los defectos técnicos en el cierre de la laparotomía, como la utilización de suturas inadecuadas, muy apretadas o distancia incorrecta entre los puntos, presencia de cuerpos extraños, hematomas o la inclusión de un asa intestinal (pellizcamiento de esta), pueden provocar la aparición de una hernia incisional, y peor aún acompañarse de una fístula intestinal. Objetivo: Corroborar la importancia del cierre correcto y cuidadoso de la pared abdominal después de una laparotomía. Presentación del caso: Se trata de una paciente de 34 años de edad con antecedentes de tres cesáreas en un período de seis años, la última hace seis meses, que fue atendida en el Hospital Mnazi Mmoja, en Tanzania, refiriendo salida de contenido líquido amarillento a nivel de la cicatriz quirúrgica. Al examen físico se constata hernia incisional y un asa intestinal adherida a la piel con un orificio de menos de un cm por donde sale contenido intestinal amarillento. Se interviene con diagnóstico preoperatorio de hernia incisional y fístula enterocutánea; se realiza resección de 3 tres cm del íleon, anastomosis termino-terminal donde se encuentra el orificio fistuloso y hernioplastia mediante la técnica de Rives con utilización de malla de polipropileno. La evolución posoperatoria fue satisfactoria. Conclusiones: Se refuerza la afirmación que es de suma importancia el cierre correcto y cuidadoso de la pared abdominal después de una laparotomía, sobre todo si se realiza con urgencia(AU)


Introduction: In the closure of the laparotomy, technical defects such as the use of inadequate sutures, too tight or incorrect distance between stitches, the presence of foreign bodies, hematomas or the inclusion of an intestinal loop (pinching) can cause the appearance of an incisional hernia which, even worse, can be accompanied by an intestinal fistula. Objective: To corroborate the importance of the correct and careful closure of the abdominal wall after performing laparotomy. Case presentation: A 34-year-old patient with a history of three previous cesarean sections in a six-year period, the last one performed six months, was treated at the Mnazi Mmoja Hospital in Tanzania when she reported the release of a yellowish liquid content at the surgical scar level. Physical examination revealed an incisional hernia and an intestinal loop attached to the skin with a hole of less than one cm through which yellowish intestinal content was coming out. She was operated on with the preoperative diagnosis of incisional hernia and enterocutaneous fistula; a three-cm resection of the ileum, end-to-end anastomosis where the fistulous orifice was located, and hernioplasty were performed using the Rives technique with polypropylene mesh. The postoperative evolution was satisfactory. Conclusions: The assertion that correct and careful closure of the abdominal wall after laparotomy is of utmost importance, especially if it is performed urgently, is reinforced(AU)

RESUMEN Antecedentes: La hernioplastia inguinal es el procedimiento quirúrgico más frecuentemente realizado dentro de la Cirugía General. Se realizan anualmente 20 millones de hernioplastias; la técnica de Lichtenstein y la transabdominal preperitoneal (TAPP) mediante cirugía laparoscópica son las más utilzadas. Objetivo: El objetivo del presente estudio es valorar la factibilidad de la realización del TAPP, en un centro universitario, por parte de residentes y cirujanos jóvenes en formación. Se comparó dicho procedimiento con la técnica de Lichtenstein valorando los resultados en los primeros 30 días del posoperatorio. Las principales variables para estudiar fueron: complicaciones, dolor posoperatorio, estadía hospitalaria y costo del procedimiento. Como variables para estudiar, a largo plazo, se compararon la recidiva herniaria y el dolor crónico. Material y métodos: Se realizó un estudio observacional comparando dos técnicas quirúrgicas utilizando la base de datos del Servicio con información adquirida de forma prospectiva. Se analizaron 80 pacientes, divididos en dos grupos: hernioplastia de Lichtenstein (grupo1) y hernioplastia por TAPP (grupo 2), durante el período comprendido desde mayo de 2015 hasta mayo de 2019, en dos Centros Universitarios de Montevideo Uruguay (Hospital de Clínicas y Hospital Español). Resultados: No hubo diferencia significativa en la estadía hospitalaria y complicaciones posoperatorias. El grupo del Lichtenstein presentó una recidiva herniaria. La hernioplastia laparoscópica presentó un tiempo operatorio promedio de 20 minutos más y el costo de materiales fue mayor. Si bien el dolor posoperatorio en las primeras 24 horas fue mayor en TAPP, el dolor a las 48 horas, 7días, un mes, tres meses y seis meses fue similar en ambos grupos. Conclusión: La técnica de TAPP, para la hernia inguinal primaria unilateral, es factible de ser realizada por cirujanos jóvenes o en formación, con un porcentaje de complicaciones, dolor posoperatorio y recidivas similares al Lichtenstein, aunque con un costo de materiales y tiempo operatorio levemente mayor.


ABSTRACT Background: Inguinal hernia repair is one of the most common procedures in general surgery. Each year, 20 million surgeries for inguinal hernia repaired are performed; the most common techniques used are the Lichtenstein procedure and the transabdominal preperitoneal (TAPP) laparoscopic approach. Objective: The aim of the present study is to evaluate the outcomes of the TAPP approach performed by residents and young surgeons in training in a university center. TAPP was compared with the Lichtenstein and the outcomes at 30 days were compared. The variables considered were complications, postoperative pain, length of hospital stay and procedure-related costs. The long-term variables analyzed were recurrence and chronic pain. Material and methods: We conducted an observational study comparing two surgical techniques, using prospectively acquired information from the database of our department. The cohort was made up of 80 patients undergoing inguinal hernia repair in two university-based hospitals in Montevideo, Uruguay (Hospital de Clínicas and Hospital Español), between May 2015 and May 2019. The patients were divided into two groups: Lichtenstein procedure (group 1) and TAPP procedure (group 2). Results: There were no significant differences in length of hospital stay and postoperative complications. One patient in the Lichtenstein group presented hernia recurrence. Mean operative time was 20 minutes longer with laparoscopic hernia repair, and the cost of the materials was higher. Postoperative pain at 24 hours was greater in the TAPP group, but was similar at 48 hours, 7 days, one month, three months and six months in both groups. Conclusion: The use of TAPP technique for unilateral primary inguinal hernia is feasible to perform by young surgeons or surgeons in training, and the percentage of complications, postoperative pain and recurrences is similar to that of the Lichtenstein technique, although the cost of materials and operative time are slightly higher.

Introducción: En los últimos años, la aplicación de la cirugía laparoscópica ha permitido ofrecer una nueva expectativa en el tratamiento de la hernia inguinal, ya que no solo permite la colocación de una malla sin tensión, sino que además permite colocarla en la auténtica zona débil de la región inguinal: su pared posterior. Objetivo: Mostrar los resultados obtenidos con la implementación de la hernioplastia laparoscópica como tratamiento alternativo en la reparación de las hernias inguinales. Método: Se realizó un estudio cuasi experimental en el servicio de Cirugía General del Hospital General Docente "Comandante Pinares", durante el período comprendido de junio de 2015 hasta mayo de 2017, con una muestra de 27 pacientes. Se controlaron variables como edad, tipo de hernia inguinal, complicaciones del proceder, recidivas del tratamiento, así como la efectividad del proceder en comparación con técnicas convencionales. Resultados: La hernia inguinal unilateral fue la más frecuente tratada, las complicaciones estuvieron dadas por la inguinodinia y rechazo a la malla. Disminuyeron variables como pérdidas sanguíneas, tiempo quirúrgico, la efectividad se consideró buena en la mayoría de los casos. Introduction: In recent years, the usage of laparoscopic surgery has offered new expectations regarding inguinal hernia treatment, since it not only allows the placement of a mesh without tension, but also allows it to be placed in the authentic weak area of the inguinal region, its posterior wall. Objective: To show the outcomes obtained with the implementation of laparoscopic hernioplasty as an alternative treatment in inguinal hernias repair. Method: A quasiexperimental study was carried out, during the period from June 2015 to May 2017, in the general surgery service of Comandante Pinares General Teaching Hospital, with a sample of 27 patients. Variables such as age, type of inguinal hernia, procedural complications, treatment recurrences, as well as the effectiveness of the procedure were controlled in comparison with conventional techniques. Results: Unilateral inguinal hernia was the most frequently treated. Complications were caused by inguinodynia and mesh rejection. Variables such as blood loss and surgical time decreased. Effectiveness was considered good in most cases. Conclusions: Laparoscopic hernioplasty is shown to have greater biological, social and economic advantages(AU)

ABSTRACT

Amyand's hernia refers to a rare occurrence in which the vermiform appendix, either inflamed or normal, happens to be found in an inguinal hernia sac. Due to its rarity and unspecific clinical evidence, it is most commonly presented as an intra-operative finding. A laparoscopic approach becomes both a way to confirm the diagnosis and a therapeutic tool. Case report: We hereby report a case of a 62-year-old patient presenting with an asymptomatic bilateral inguinal hernia, previously treated on his right side in 2011 with an open approach. The elective laparoscopic surgery, during the right groin exploration, revealed a vermiform appendix, with no signs of inflammation, within the hernia sac. . A prosthetic laparoscopic hernioplasty without appendicectomy was performed and both early outpatient follow-up and 30-day outcome demonstrated excellent recovery. Conclusions: Appendicectomy, when necessary, and primary hernia repair at the same time can be safely performed by laparoscopy which may be considered an advantageous management giving its role in diagnosing, in confirming an Amyand's hernia, in exploring the abdominal cavity and in being a therapeutic tool at the same time(AU)


La hernia de Amyand se refiere a una ocurrencia rara en la que el apéndice vermiforme, ya sea inflamado o normal, se encuentra dentro de un saco inguinal herniario. Debido a su rareza y evidencia clínica inespecífica, se presenta más comúnmente como un hallazgo intraoperatorio. Un abordaje laparoscópico se convierte tanto en una forma de confirmar el diagnóstico como en una herramienta terapéutica. Caso clínico: Presentamos un caso de un paciente de 62 años que presenta una hernia inguinal bilateral asintomática, previamente tratada en su lado derecho en 2011 con un abordaje convencional abierto. La cirugía laparoscópica electiva, durante la exploración de la ingle derecha, reveló un apéndice vermiforme, sin signos de inflamación, dentro del saco de la hernia. Se realizó una hernioplastia laparoscópica protésica sin apendicectomía y tanto el seguimiento ambulatorio temprano como el resultado a 30 días demostraron una excelente recuperación. Conclusión: La apendicectomía, cuando es necesario, y la reparación de la hernia primaria al mismo tiempo se pueden realizar de forma segura por laparoscopia que puede considerarse una gestión ventajosa dando su papel en el diagnóstico, en la confirmación de una hernia de Amyand, explorando la cavidad abdominal y siendo una herramienta terapéutica al mismo tiempo(AU)

BACKGROUND: Chronic pain after tension-free inguinal hernia repair is a common postoperative complication. An important cause of chronic pain is nerve injury. OBJECTIVE: To observe the effect of iliohypogastric nerve preposition on chronic pain after inguinal hernia repair and analyze its mechanics. METHODS: Totally 76 male inguinal hernia patients, from August 2013 to November 2016, aged 36-95 years in Second People’s Hospital of Lianyungang were selected. The patients were all treated with large-size polypropylene non-absorbable mesh insertion. The iliohypogastric nerve was placed in front of the mesh. Pain was followed up at 3, 6, and 12 months after operation. This study was approved by the Ethics Committee of Second People’s Hospital of Lianyungang. RESULTS AND CONCLUSION: (1) No chronic pain or recurrence was found in the 76 patients at 3, 6 and 12 months after surgery. (2) The pressure included vertical pressure and tangent pressure formed from intra-abdominal pressure on abdominal wall, directly affecting abdominal wall and artificial repair materials. This was the basic cause of the relative mechanical relationship between nerve and mesh. (3) The results showed that the tension-free inguinal hernia repair with the iliohypogastric nerve preposition could reduce the incidence of postoperative chronic pain, which may be related to the effect of postoperative abdominal pressure on tissue repair, especially the repair around the mesh.

Background: The repair of inguinal hernias has seen an evolution over the past few decades and more research on the same is still underway. Though laparoscopy has gained widespread acceptance in today’s era of surgery, there is still a debate between laparoscopic and open hernia mesh repair.  Methods: A randomized prospective study was conducted at a tertiary care teaching hospital to compare laparoscopic hernioplasty and Lichtenstein’s open mesh repair. The study consisted of 70 subjects with unilateral or bilateral inguinal hernia and they were randomly allocated into either group. Various parameters like duration of surgery, intra and post-operative complications, post-operative pain, recurrence, stay in the hospital and resumption of daily activities were compared.Results: Out of the 70 patients, 35 underwent laparoscopic hernioplasty and 35 underwent open hernia repair. The mean operative time for laparoscopic hernioplasty (unilateral 63.44mins, bilateral 123.80mins) was greater than open hernioplasty (unilateral 47.35mins, bilateral 90.42 mins). Post-operative complications, like wound infection, seroma formation and urinary retention were noted more in the open hernioplasty group. The mean pain score for laparoscopic hernia repair was lower than open hernia repair on postoperative day 3 and 7. The average duration of hospital stay was 3.5 days in laparoscopy group and 6 days in open group. The mean duration for resumption of daily activities was 4.8 days following laparoscopic hernioplasty and 8.1 days following open hernioplasty.Conclusions: Laparoscopic hernioplasty is more beneficial than Lichtenstein’s open hernia mesh repair as it is safer, with faster recovery, lesser post-operative complications and reduced morbidity.

Background: Incisional hernias repair being done in large numbers there is still not a consensus about the best repair. Very few studies have been done on comparison open and laparoscopic incisional hernia repair.Methods: A prospective, cross-sectional study was undertaken in Department of Surgery of Civil Hospital. The study included total 50 patients, out of which 25 patients underwent open approach and rest of 25 patients, underwent laparoscopic approach. Patients were assigned to both the groups randomly.Results: Pain, duration of post-operative stay, and return to routine work is earlier in patients with laparoscopic repair mainly due to decreased pain, fewer complications, early mobility and faster return of bowel movements. Laparoscopic repair is more expensive and operative time is more as compared to open method.Conclusions: Keeping in view the advantages and limitations of laparoscopic repair, the choice among two surgical modalities should be made on a case to case basis depending on patient preference and characteristics.

Background: Inguinoscrotal swellings are the most common presenting complaints in the surgical out patient department and surgeries for the same are the most commonly done elective procedures.However meticulous the surgeon is, there is always some amount of scrotal oedema. Our study is intended to use a simple and novel technique by applying a scrotal hitch after the surgery to prevent the scrotal oedema. The aim of the study was to evaluate the effectiveness of a scrotal hitch in reducing the scrotal oedema and pain postoperatively.Methods: In this study all subjects undergoing inguinal hernia surgeries at undergoing at R.L. Jalappa Hospital, Kolar from June 2019 to August 2019 were included. Of the 60 subjects, 30 in group A were given scrotal hitch and subjects in group B were given scrotal support alone. The incidence of scrotal oedema and postoperative pain and development of any complications will be compared in both the groups.Results: A total of 60 subjects were included in this study. Mean age of patients in group A is 45.63±21.745 and group B is 41.43±24.579. 2 patients with scrotal hitch developed scrotal edema whereas 6 patients developed scrotal edema in group B. The post-operative pain score was lower in patients with scrotal hitch.Conclusions: Scrotal hitch is a simple and effective technique in preventing post-operative oedema and pain. The results of this study would encourage surgeons to take up this simple procedure to prevent the commonest complication of inguino-scrotal surgeries.

Background: Mesh hernioplasty in patients undergoing emergency inguinal hernia repair is considered practically, irrespective of complications. The main aim of this study was to assess the morbidity of Lichtenstein mesh hernioplasty in treating obstructed inguinal hernias. Primary outcome measures were post-operative wound site infection, seroma formation, length of hospital stay, hanging testis, testicular infarct, inguinodynia and recurrence.Methods: This study was undertaken in the department of General Surgery, SMIMER, Surat, Gujarat, India from August 2016 to July 2019. Fifteen patients were operated and included in the study. All patients underwent standard Lichtenstein mesh hernioplasty for obstracted inguinal hernia repair in emergency operating room.Results: 5 patients (33%) developed wound site infection, 4 patients (27%) developed inguinodynia, 2 patient (13%) developed seroma formation, 1 patient (6%) developed hanging testis. 1 patient (6%) developed testicular infarct. Average postoperative hospital stay was 5.6 days (range =2-18 days).Conclusions: Mesh repairs can be safely performed in emergency inguinal hernia repair with acceptable morbidity.

Introduction: The intention of axillary lymph node dissection (ALND) is to stage the axilla precisely for prognostic information.This study is to assess the possibility of skip lesion in Level 3 in the absence of disease in Level 1 and 2 which may help inundertaking randomized controlled study to avoid Level 3 nodal dissection in our patients.Materials and Methods: Retrospective analysis of 60 patients who underwent surgery for invasive breast cancer from October2013 to October 2019 in the Department of Surgical Oncology, Government Thoothukudi Medical College Hospital, Thoothukudi,was performed.Results: About 33.3% of patients (20) were disease free in the axilla and the remaining 66.7% (40) had nodal involvement. Ofthose 39 patients who had nodal involvement in Level 1 and 2, 15 were found to have the disease in 4 or more nodes, 24 hadthe disease in <4 nodes. Totally 16 patients had metastases in Level 3 nodes. There is a 60% chance of involvement of Level3 when there is 4 or more nodal positivity in Level 1 and 2 and it drops to 25% if the involved nodal count becomes <4. Onepatient had skip lesion in Level 3 (4.8%) without disease in Level 1 and 2.Conclusion: Since there has been a dearth of randomized studies about levels of nodes to be addressed in ALND and studiesabout skip lesion in Level 3 from our country, we urge the need for more studies probably multicentric, regarding the extent ofALND. Until then, it may be fruitful to do complete ALND up to Level 3 for the better staging of the axilla.

Background: Paraumbilical hernia is a common disease and its repair is one of the most performed surgical interventions worldwide. The most important techniques in its repair are hernioplasty and herniorrhaphy and both differ significantly.Methods: We conducted a prospective randomized controlled trial to compare hernioplasty versus herniorrhaphy alone in small-sized paraumbilical hernia as regards local wound complications e.g., seroma and wound infection in addition to recurrence rates, operation time and overall cost, by comparison between two groups each comprising 50 patients.Results: There was no significant difference between both techniques regarding hernia recurrence as well as wound complication. On the other hand, herniorrhaphy reduced significantly incision size, time of operation and overall cost.Conclusions: Our study revealed that herniorrhaphy alone is better in small-sized paraumbilical hernia repair as it showed significant correlation with decrease incision size, time of operation and overall cost.

Background: Inguinal mesh hernioplasty is one of the common procedures performed all over the world. It can be done either through open or laparoscopic techniques. The aim of this study was to compare the outcomes of Lichtenstein tension free hernioplasty versus laparoscopic transabdominal pre-peritoneal (TAPP) mesh repair considering, duration of the surgery, hospital stay, and duration to resume normal activity, degree of postoperative pain, wound infection, recurrence and complications.Methods: Adult patients presented to the general surgical OPD, with the diagnosis of inguinal hernia underwent either Lichtenstein repair or laparoscopic repair by TAPP.Results: Patients in Group A (open-repair) had significantly greater level of local pain during rest and during routine activities than those within Group B (laparoscopic group) during the postoperative period assessed on the visual-analogue scale. Mean operative time for open hernia repair was 43.7 minutes and for laparoscopic hernia repair was 59.03 minutes and the difference were statistically significant (p=0.0001). The mean duration of hospital stay for open hernia repair was 2.16 days and that for laparoscopic hernia repair was 1.08 days with a (p=0.00001) which was statistically significant. The time to resume routine activities was much shorter among Group B patients than patients in Group A. Only one recurrence (3.3%) was seen in Group B after 6 months follow up.Conclusions: It is concluded that laparoscopic TAPP repair of inguinal hernia in adults is safe and preferred operation as compared to open inguinal hernia repair.

Background: Inguinal hernia repair is one of the most commonly performed procedures by general surgeons. Cyanoacrylate is the generic name for a family of fast acting adhesives. The aim of the present study done in Department of General Surgery, Safdarjung Hospital, New Delhi was to compare the newer emerging technique of mesh fixation.Methods: A total of sixty patients were included in the present study and were allotted in case and control group randomly by sealed envelope technique. In case (study) group, all the patients underwent mesh fixation by cyanoacrylate glue and in control group, by prolene 3-0 sutures.Results: Most frequency in age group 31-40 yrs, males:females ratio >1 and right sided inguinal hernia was more common. Bi-lateral hernia was common in elderly. Indirect: direct ratio 4.5:1. Operating time period for the patients of the case (study) group is less than control group. P value of post-operative pain in immediate post-operative period (day 1 and 2) and POD 30, 60 and 90 was not of clinical significance whereas the p-value on 6,120,150 and 180 post op day was of clinical significance. In our study, there was a case of incidental observation: a) reaction due to use of cyanoacrylate glue, b) rejection of mesh for which mesh had to be removed.Conclusions: There is no statistically significant difference between mesh fixation with cyanoacrylate glue and mesh fixation by prolene suture techniques in immediate post-operative pain. Statistically significant difference favoring mesh fixation by cyanoacrylate glue technique was seen with respect to operating time and post-operative groin pain with increasing post-operative duration.
